About Me
My name is Emma Hazelton, and I am an Occupational Therapist with over 17 years' experience. I have extensive professional and lived experience with supporting complex NDIS participants.
I focus exclusively on Functional Capacity Assessments (FCA reports) for adults and children with psychosocial, cognitive, intellectual, and behavioural challenges.
I provide clear, detailed, NDIS-aligned reports that maximise funding outcomes and reflect the participant's true daily needs.
I am based in Redlands (Cleveland), Brisbane and offer both in-person and telehealth assessments.
Referrals are welcome directly via email or phone. General Capacity is available from August - urgent reports can normally be fitted in.

How It Works – Simple 3-Step Process
Step 1: Get in Touch
We start with a brief phone or email consultation to understand:

Step 2: Assessment (In-Person or Telehealth)
I conduct a full Functional Capacity Assessment tailored to your situation. This includes:

Structured interviews with you (and/or your parent, carer, or partner)

Review of existing reports, school/therapy notes, carer letters
-
Assessments (e.g., Vineland-3, WHODAS, eCANS, PEDICAT – depending on need)

Allied health professionals already involved in your care 💰 Rebates:

If you’re eligible, I can offer Medicare CDM rebates (up to 5 sessions per year with a valid GP referral)

Step 3: Report + Ongoing Support
You’ll receive a clear, detailed NDIS-aligned report within ~2 weeks (depending on complexity and urgency). My reports:

Are fully aligned with current legislation (Oct 2024 update) with each section justifying needs and recommendations against the 'Reasonable and Necessary' Criteria.

👥 If you don’t have a Support Coordinator:
I continue to check in with you or your nominee after the report is submitted — until your plan meeting is held. I can:

Provide extra documentation or clarification if requested by the NDIA

💰 How much does a Functional Capacity Assessment (FCA) cost?
Most FCA reports take 13–18 hours, with an average of 15 hours total @ $193/hr, including:

📄 How do Medicare CDM rebates work for NDIS Access reports?
If you're applying to access the NDIS and don’t yet have funding:

I offer a mini-FCA and Access report tailored to meet NDIA criteria

This takes around 4–5 hours in total and includes:

🏥 If your GP provides a Chronic Disease Management Plan (CDM), you can claim rebates for up to 5 sessions. I can:

Yes — I provide basic AT and consumables recommendations, including:
